The enrollment form includes general information about the workshop, including workshop materials and costs.
Print and mail the completed form to us. You can also reserve a spot prior to mailing your enrollment form for a workshop over the phone or email to the workshop coordinator, Amelia Garripoli, (360) 582-0697, poetread@gmail.com.

Reserved spots will be held 7 days and then released if payment has not been received.
Classes with insufficient enrollment by Sept 10 will be cancelled.
If you have any trouble downloading the enrollment form, call to have one mailed to you.
